Shanghai Yueyang Hospital group portrait of the anti-new crown: the daily emergency treatment of a thousand people, no one stay out of the way
“The patient’s blood oxygen saturation is only 73%, immediately tracheal intubation!”
On the morning of Jan. 3, Qian Yiming, director of the emergency medicine department of Yueyang Hospital, was in the resuscitation room directing the rescue of a 65-year-old man who had been infected with a new crown and whose old COPD (chronic obstructive pulmonary emphysema) had worsened.
In the resuscitation room, the medical and nursing staffs were already busy, most of the medical and nursing staffs involved in the resuscitation were “Yang” Kang back on duty or with “Yang” on duty, but at this critical moment of the race with death, no one stayed away. After several hours of intensive resuscitation, the patient’s vital signs are now stable.
As the number of new infections continues to rise, Shanghai’s medical institutions face a serious challenge. In Yueyang Hospital of Traditional Chinese Medicine (hereinafter referred to as Yueyang Hospital), a group of health care workers overcame many difficulties and worked day and night on the front line of treatment.
The emergency department receives about 1,000 visits per day, and a critical care team is formed.
The data shows that up to now, Yueyang Hospital receives about 1,000 emergency room visits per day, and the number of ambulances is 80-100. This has become a “battlefield”.
At present, there are data showing that Yueyang Hospital’s emergency department receives about 1,000 visits per day.
Late on January 2, a patient with upper abdominal pain came to Yueyang Hospital’s emergency room. 73-year-old Xu was indifferent, pale, and sweating coldly, with a blood pressure of only 80/50mmHg. He had just arrived at the emergency room with hypotension and other shock symptoms, and the CT examination elevated the perforation of the digestive tract.
At this moment, the deputy chief physician of general surgery, Dr. Sun Jiuyun, is the second day of “Yang”, with fever and symptoms of nausea and vomiting, but still on duty. In the examination for the old man Li, he vomited once again.
“The patient’s condition is urgent, life-saving is important, prepare for surgery immediately!” Sun Jiuyun immediately contacted the operating room of the Department of Anesthesiology, and after perfecting all the emergency surgery preparations, old man Xu was escorted into the operating room at the first time. Sun Jiuyun, who was still feverish, quickly adjusted his status and fought for time to successfully perform the gastric perforation repair for Li. Once the operation was over, Sun Jiuyun, who had just gotten off the operating table, threw up again.
Since the beginning of this round of epidemic, Yueyang Hospital immediately set up a “special team for emergency critical care referral” and formed a “medical treatment team for severe infection of the new coronavirus”. The Medical Affairs Department, together with the Nursing Department and the Department of Emergency Medicine, coordinated the medical and nursing resources, inpatient bed resources, drugs and equipment resources of the hospital to orderly classify and treat emergency patients, ensuring that the transfer of emergency critically ill patients from the emergency clinic, fever clinic and all ward buildings was carried out efficiently.
The expert team carries out daily rounds in each ward building and discusses acute and critical cases to ensure smooth checkups, discussions and treatment after the admission of acute and critical patients in each ward building. The Department of Emergency Medicine has set up a second emergency ward to refer emergency critically ill patients by combining with a mixed team of clinicians from dermatology, gynecology, ophthalmology and other departments.
Medical personnel are treating patients with new crowns.
“120 is coming, prepare for resuscitation”, “the patient’s oxygen saturation is low, cooperate with intubation”, “open intravenous access quickly!” The patient’s oxygen saturation is low, cooperate with intubation, open intravenous access!”, “Send this patient to do CT.” …… Luo Qing, deputy chief nurse of the emergency medicine department, comes to the emergency lobby before 7:00 every day and stays busy until late at night. She is like a “lobby manager”, to ensure the smooth flow of green channels, rescue critical patients, arrange patient consultation, examination, maintain the order of the emergency, hissing and screaming every day, basically no time to eat lunch.
“At present, emergency patients are still increasing, we are all psychologically prepared, almost all of our doctors and nurses in the emergency department are still holding on to their posts. The workload is really too heavy, and some colleagues have also developed symptoms of viral pneumonia and had to rest.” Chief Nurse Cao Wei-ling support the provinces back immediately into the hospital battle, “but the whole hospital colleagues are here in our emergency support, I believe we can go through this difficult time together.”
Fever clinic: four to five hundred people visit every day, patients mostly have underlying diseases
Chen Yang, a doctor in the Department of Emergency Medicine at Yueyang Hospital. He has been working in the fever clinic and infection ward since March 2022. In recent times, the fever clinic at Yueyang Hospital has been seeing 400-500 patients per day. During the most stressful days, Chen Yang worked continuously for 5 days, with a daily working time of 14-17 hours, until the first batch of “Yang Kang” colleagues returned to work. And after a short recuperation after his own “Yang”, he returned to work.
“Most of the elderly patients in the fever clinic ward almost always have obvious underlying diseases, such as cerebral infarction, heart failure, respiratory failure, epilepsy, etc.. With patients in serious condition, unable to take care of themselves, and family members unable to accompany them, the pressure and difficulty of nursing work is very high.” Nurse Zhu Yewei, who has been quietly staying at the fever clinic, the “outpost” of the epidemic since the 2020 epidemic, and her colleagues have been guarding every patient with their excellent nursing skills and high sense of responsibility.
Lv Jiajia, an attending physician at Yueyang Hospital’s pediatric fever clinic, was a one-man fever clinic for 48 hours at the most difficult time. “Colleagues are ‘yang’, they all have heavier symptoms than me, but they all came right back when they got a little better.”
Pediatric outpatient pre-screening nurse Zhang Wanping used the outpatient clinic lunch break to take the initiative to help in the emergency department, “the emergency department now has a huge workload, it is incumbent to help.”
In the emergency room of Yueyang Hospital, medical and nursing staff are helping patients with infusion treatment.
Anesthesiology, laboratory and other departments of medical care to support the new crown rescue and treatment front line
“Ding bell, ding bell ……” the last day of 2022, anesthesiology attending doctor Zhu Ping received a call from the emergency resuscitation room at 8 a.m.: “Anesthesiology, quick, emergency room intubation! ” The cross-year shift started with a busy resuscitation.
On this day, Zhu Ping and his colleagues completed 11 surgeries, 5 cases of emergency tracheal intubation, 8 cases of deep venipuncture, and also supported the fever clinic. In this special period, the anesthesiologists rushed from “behind the scenes” to the front line of treating critical patients.
Zhu Ping said, they always believe that the efforts of doctors can make patients recover as soon as possible, as soon as possible to return to normal life.
Xu Min, the head of Yueyang Hospital’s clinical examination and emergency outpatient team. During this three-year period of the epidemic, he has always been on the front line, and the number of days he has been home can almost be counted on his fingers. The “first hospital 24-hour nucleic acid sampling site in Shanghai”, the permanent nucleic acid sampling site and the alteration and improvement of the fever clinic and emergency outpatient testing service capacity …… Where there are difficulties, Xu Min’s figure can be seen. Since the current round of the epidemic, many of the staff of the testing department are insisting with illness, or just improved, they are cloaked in battle, Xu Min has not gone home for nearly a month.
On the basis of the national program, Yueyang Hospital also actively uses Yueyang’s special epidemic prevention preparations such as Jin Ju Qing Run Granules and Jin Qi Qing Shu Granules for treatment. Doctors from the departments of Tui-Na, Acupuncture, Dermatology, Pain, and the Center for the Cure of Disease were the first to fill up the clinical treatment in the emergency and fever clinics. Rehabilitation therapists went to all wards of the hospital to assist medical and nursing staff to guide patients and carry out “prone ventilation” to help patients recover better and faster.
